Ohio Tuition Trustissue tax return?

I came to a question on my return "qualified education distributions," I want to record. I received money from a 529 savings plan qualified last year for college.

He requested information from the payor. Now is the payer to the person who put the money in the account (eg my father), or is it the current distributor? (In this case, the authority of the Ohio tuition trust).

He also asked a customer id, where can I find it?

You should have received a Form 1099-Q (or replacement statement similar) which will have all the necessary information about it.

It is the Ohio Tuition Trust Authority. If you do not have a document showing their tax identification number, you should be able to search and find them.
